CFD Facilities
For all CFD shifts, please check in and out using the Skedulo app.
Time Record
Fill out a time record, and take a picture of it. Upload the photo to the corresponding Job in the Skedulo app.

All lunches should be recorded on the time record itself. You do not need to check in or out of the app for those.
Your time record needs to be signed. Please leave the physical copy on site for staff to sign and review.

Schools / School Districts
For all school shifts, please check in and out using the Skedulo app.
Time Record
Fill out a time record, and take a picture of it. Upload the photo to the corresponding Job in the Skedulo app. All shifts should be initialed—you will be guided by the school on who the proper staff member is for this.

All lunches should be recorded on the time record itself. You do not need to check in or out of the app for those.
All time records are shared with the client. If you work with different school districts, each should have their own individual time record.
Some school districts require the physical time record sheet to be left on site. If requested, please leave the physical time record sheet on site for the school staff to review.
Time Records
While the majority of schools accept NurseRegistry’s time record, some may require their own. You will always be notified if there is a specific document you need to use.

All Other Facilities
For all facility shifts, please check in and out using the Skedulo app.
Time Record
If you are taking lunch, fill out a time record and have it signed by a facility staff member. Take a picture of the time record and upload the photo to the corresponding Job in the Skedulo app.

If you are not taking lunch, you do not need to upload a time record in the Skedulo app.

Additional Instructions
One time record is due for each facility. Each facility must be on a separate time record because it will be sent to the facility for review.
Some facilities may have paper or computer charting. We will let you know beforehand what kind of documentation is required for a particular client, in addition to the time record.
Private Clients
For all Private Clients, please check in and out using the Skedulo app.
Care Notes
Fill out your Care Notes, and take a picture of it. Upload the photo to the corresponding Job in the Skedulo app.
Some private clients may have a care binder for charting in-home. If a care binder is present in the home, notes should be left in the binder and a photo should be taken. The physician and case manager may receive a copy of the notes upon request.
All care notes must be uploaded within 24 hours of the visit.
Care notes can be downloaded here. The only exception is Sunday visits, which must be submitted by Monday morning at 8 am for payroll.
Additional Instructions
- Care notes should be dated with time-in and time-out.
- They must be signed with the nurse licensure (RN/LVN).
Important
Please email info@nurseregistry.com if you are confirming assignments directly with the client – we will need to add these jobs so you may access them in Skedulo.
Pharmacy Clients
For all Pharmacy Clients, please check in and out using the Skedulo app.
Pharmacy Paperwork
Each pharmacy has its own proprietary documentation, which you will be informed of prior to your visit.
Notes must be filled out completely, as every field is important. If there is a client signature line, the client must sign. The pharmacy and physician may receive a copy of the notes upon request.
Fill out your notes, and take a picture of them. Upload the photo to the corresponding Job in the Skedulo app. Notes are due within 24 hours of the visit. The only exception is Sunday visits, which must be submitted by Monday morning at 8 am for payroll.
Additional Instructions
- Nurses are responsible for scheduling the next visit. If not in the notes, then let us know in the email.
- If not scheduled at time of visit, contact us so we can notify the pharmacy when to send supplies to the patient.

How to Upload a Time Record to a Job
Here are instructions on how to upload a time record to a Job in the Skedulo app.
1. Open the specific Job you would like to attach your time record to. Click on the paperclip icon in the top right.

2. From here, you have two options: camera or browse.
- Camera: You will be prompted to allow access to all photos, selected photos or none. Please allow all or a selection. You now access to the camera app, where you can take a photo of the time record to upload.
- Browse: Click the browse button to be able to select and upload a copy of the time record. You can take a picture of your time record outside of the app, or scan a copy to your phone so that it may be accessible here. Select your file and tap the add button.

3. To delete an attachment, choose one of the following options.

- Tap the photo to open it, then tap the trash can icon in the top right. Tap yes to confirm.

- You may also hold your finger on the file to delete. A prompt asking you to confirm will appear. Tap yes to confirm.

4. Once your file is attached, tap the back button. A number on top of the paperclip icon will indicate how many files you have uploaded to the job. That’s it! The file has been successfully saved to the Job.
